PAYMENT POLICY  

As of January 1, 2007 Dr. Mallett is out of network with all insurance carriers (except Medicare and some Medicare replacement plans). Payment is due in full at time of service. Please provide the receptionist with your health insurance information when you check in. After your visit, a completed claim form and receipt will be provided for you to mail to your insurance company. You will be reimbursed directly for any benefits due.

We accept cash, checks, MasterCard, VISA, and Discover credit cards.

Lab and X-Ray

Any Lab and X-Rays ordered for you by Dr. Mallett are performed at an outside facility. Should you have any questions regarding insurance or billing, please contact them.

Completing forms for camps, schools, employers, license renewals, etc.
(If an office visit is necessary to complete specific information on these forms, there will not be an additional charge to complete form)

Writing and dictating letters or reports for airline reimbursement in the event of illness; for employers requiring documentation of an illness; excuse from jury duty; medical summaries for travel; or other agencies if necessary.
